Test Proctoring

Students coordinate between their institution and our library. Our Test Proctor will review institutional guidelines before scheduling a test date. (We may not be able to meet technology requirements set by some online institutions.)

–The student is responsible for determining the requirements for, and the materials needed to complete the test
–The student is responsible for emailing the test requirements (given to the student from the school) along with the requested test date to: jolar@pekinpubliclibrary.org AT LEAST 2 weeks prior to testing deadline, to arrange a time, and insure all paperwork can be completed 
–The student provides a pre-paid envelope for a written test that requires return via mail
–The student must provide current photo ID, and the name on the ID must match the name on the testing materials
–The student must provide all of the supplies needed to complete the test, including: headphones, pencil/paper, laptop, etc

Test Proctor will enforce time limits and will monitor testing as their library duties allow. The library is not responsible for the content of tests once the test has been sent to the institution upon completion. The library cannot assume responsibility for completed tests that are not received by the education institution deadline. The student should contact the institution to determine if the completed test was received. Tests are proctored during normal library hours.

Passport Processing

  • DS-11 Application: Complete sections 1-22 in BLACK INK with NO MISTAKES; Do Not Sign the Application. Social Security Number is a requirement. All applicants applying for a passport must be present. Forms are available at the library or downloaded at travel.state.gov
  • Proof Of Citizenship: Certified Copy of a Birth Certificate containing the full names of one or both parents, or current/previous U. S. Passport, or Naturalization Certificate. Hospital Issued Birth Certificates are not accepted.
  • Proof of Identity, which can be a Valid U.S. Driver’s License, Military or Government-Issued ID. (We will make a copy of your ID to submit with the application.)
    Out-of-State Driver’s Licenses require a second Photo ID. A photocopy of BOTH parental ID’s are to be submitted with a child’s application.
  • One Passport Photo, which can be taken at your appointment for $10.

Additional Needs/Children:  IN ADDITION to other requirements, please note:

  • Children 15 and under: Both parents must be present with the child/children. If one parent cannot appear, the absentee parent must submit a notarized statement of consent along with a photocopy of the front and back of the driver’s license. A photocopy of both parents ID’s are submitted with each child’s application.
  • 16 & 17 Year-Olds: One parent must accompany a 16 or 17 year old applying for a passport. They must submit a certified copy of the birth certificate for a new passport or a current/previous passport.
